Care Coordinator

Hourly Rate: £14.50  
Hours: Full-time, including a mix of day and night shifts

Our client is looking for a proactive and experienced Care Coordinator to join their team in a welcoming and supportive care home environment. With a focus on delivering high-quality, person-centred care, this role is perfect for someone with a Level 3 Health and Social Care qualification and previous experience as a Care Coordinator or in a similar position within a nursing or care home setting.

What we’re looking for:

  • Level 3 Health and Social Care qualification.
  • Proven experience as a Care Coordinator or in a similar role within a nursing or care home.
  • Strong organisational and time-management skills.
  • Excellent communication and interpersonal abilities.
  • A caring and empathetic approach to working with older adults.
  • Knowledge of computerised care planning systems.
  • Experience in administering medication.

Key Responsibilities

  • Care Management: Coordinate and manage person-centred care for residents, considering individual needs, preferences, and goals.
  • Shift Organisation: Plan and oversee daily care schedules, ensuring seamless care delivery across shifts.
  • Medication Administration: Safely administer and record medication, ensuring compliance with all regulations.
  • Record Keeping: Maintain accurate and up-to-date computerised care plans and documentation.
  • Staff Support: Assist with the induction, training, and supervision of support staff, fostering a positive team environment.
  • Collaboration: Liaise with healthcare professionals, residents, and families to ensure holistic care and effective communication.
